If you’ve known Thrive as Franklin’s direct primary care practice, you’ll notice something different. We’ve evolved our model — deliberately, and for good reasons.

The classical direct primary care structure got one thing exactly right: a physician should answer to her patients, not to insurers. We’re keeping that. But a single flat fee, we found, quietly works against the very thing it’s meant to protect — it pays a physician the same whether a visit takes fifteen minutes or ninety, which over time penalizes exactly the kind of thorough, unhurried care that complex patients actually need.

So we rebuilt the model around that problem. What follows is the result: a membership designed to make genuine thoroughness sustainable instead of heroic.


How Thrive works

Thrive is a membership practice. Instead of billing insurance for each visit, your care is delivered through an annual membership — which is what frees your physician from the volume-and-billing machinery that makes conventional primary care feel so rushed.

There are no co-pays, no claims, and no insurance involvement in your primary care. Most members keep their insurance for the things membership doesn’t cover — specialists, hospitals, imaging, labs, and emergencies — and simply stop using it for primary care.


Three tiers, one standard of care

Thrive offers three membership tiers. The standard of clinical care is identical across all three — the same physician, the same rigor, the same judgment. What differs is the depth and frequency of engagement: how proactively we monitor, how often we review, and how much continuous synthesis your health calls for.

Essential — for generally healthy adults who want reliable, unhurried access to a physician who actually knows them.

Proactive — for those who want structured, ongoing management and a physician who reaches out, rather than waiting to be asked.

Elite — for those managing real complexity or pursuing the most intensive, continuously integrated approach to their health, with the most direct access to Dr. Medeiros.

Memberships begin at $2,500 per year. Annual and semiannual prepayment is available at a meaningful discount; quarterly payment is also offered. We don’t offer month-to-month membership — the relationship is built on continuity, not access you switch on and off.
